·        You can get application form from the office of Gram Pradhan / and Sabhasad of Your District at free of coast.

·        Fill all the required information completely and carefully.

·        After completion of application form you can submit it to your Gram Pradhan office / Sarpanch / nambardar / Nagar Nigam commissioner for getting the pension. Monthly income certificate will be issued by Patwari. 

·        Application form also can submitted at your nearest CSC Centre.
